Adult Participants
Adult Release of Liability
Use this form for participants who have reached the age of majority in the province or territory where the rodeo activities are provided.

Use this form for participants under the age of majority. A parent or guardian must read, complete, initial, and sign the waiver on behalf of the minor participant.
